The Consulate General offers the following notary and authentication services:

General Affidavit/ Sworn Statement/ Jurat

        Requirements:

  • Personal appearance of the applicant.
  • Two (2) copies of completed appropriate forms.
  • Copy of affiant’s (applicant) passport.

Affidavit of No Relation

        Requirements:

  •  Personal appearance of the affiant (person executing/signing the document).
  •  Two (2) copies of general Affidavit form completed by the affiant.
  •  Two (2) photocopies of affiant’s passport.
  •  Two (2) photocopies of passport of the person given the visa.

Affidavit of Support and Consent/ Parental Travel Permit (Child of minority age)

       Requirements:

  •  Personal appearance of parent/s.
  •  Two (2) copies of form to be filled up by the affiant.
  •  Two (2) photocopies of affiant/s’ passport.
  • Other supporting documents.

 

CERTIFICATION

Certification for Driver’s License       

        Requirements:

  • Two (2) copies of applicant’s driver’s license.

No Objection Certificate

Requirements:

  • Copy of parent’s report of marriage (duly authenticated).
  • Copies of parents’ passport.
  • Copy of child’s birth certificate (duly authenticated).

 

SEEN AND NOTED DOCUMENTS

       Requirements:

  • Copy of the document.
  • Copy of applicant’s passport.

 

NBI FORMS

       Requirements:

  • Photocopy of completed NBI Form (Form available at the Consulate General).
  • Copy of applicant’s passport

 

DOCUMENTS REQUIRING ACKNOWLEDGEMENT (Ribbon and Seal)

Affidavit of Support and Guarantee (For visit of Family Relation)

        Requirements: (Filipino Citizen as Guarantor)

  • Personal appearance of the affiant (applicant).
  • Two (2) copies of completed Affidavit of Support and Guarantee form.
  • Two (2) copies of affiant’s passport and residence visa.
  • Copy of passport of person invited to UAE.
  • Copy of visa (if available).
  • Other related documents (salary certificate, employment contract, etc.).

Attestation 

        Requirements: (Foreign National as Guarantor)

  • Personal appearance of the applicant.
  • Affidavit or Invitation Letter executed by the person inviting, and duly attested by the Notary Public (branch of Dubai court)/ or by the applicant’s consulate.
  • Copy of affiant’s (applicant)passport and residence visa.
  • Copy of visa.
  • Other related documents.

Requirements: (Company/ Agency as Guarantor)

  • Affidavit or Invitation Letter from the company or agency ( using the company’s letterhead).
  • Copy of Company’s Trade/Commercial License.
  • Other related documents.
  • Documents must be stamped or attested first by the Chamber of Commerce (Dubai or Sharjah, depends on the location of the company).
  • After Chamber of Commerce, documents must be stamped or attested by the Ministry of Foreign Affairs (MOFA), prior to processing at the Consulate General of the Philippines.

Attestation of Documents with stamp of the UAE EMBASSY in Manila, Philippines

Requirements:

  • Copy of document.
  • Passport copy of the applicant.

Special Power of Attorney (SPA)

        Requirements:

  • Two (2) copies of completed SPA form.
  • Two (2) copies of applicant’s passport.

AUTHENTICATION OF CONTRACTS, JOB ORDERS, MANPOWER REQUESTS  (Commercial Documents)

Requirements:

  • Documents must be processed first at the Philippine Overseas Labor Office (POLO)

 

Certificate of Not the Same Person (NTSP) – Bureau of Immigration
